About 5 years ago, my company stumbled onto an incredibly effective way to generate revenue and referrals without spending a single dollar on ads and without offering coupons or discounts.
It was a simple strategy that involved sending out 1 email a week to the existing patients of the practice (no identifying info is used in these emails) with tips and techniques that are relevant to that audience. No hard marketing.
